The problem: Mother from vulnerable groups see a doctor for the first time when they give birth. More often than not, the doctor has no knowledge of the medical history of the patient and needs to find out information as fast as he can to give her the proper treatment and to make sure the birth process is the best possible for both the mother and the child. As soon as she is back in her community, nobody really keeps track of her health, nor the baby's.
The solution: Case file is a case management tool that allows social workers, nurses or other NGO personnel active in vulnerable groups to keep diaries on mothers and their children by filling in simple forms that get stored in a secure database. This way, they benefit from minimal medical supervision and the information can be shared with the doctor when a mother reaches the hospital for better intervention.
